68Ga-FAPI PET/CT as an Alternative to 18F-FDG PET/CT in the Imaging of Invasive Lobular Breast Carcinoma.

Accurate staging of invasive lobular carcinoma (ILC), a subtype of breast cancer, is vital for effective clinical management. Although 18F-FDG PET/CT is a commonly used tool, its efficacy varies across different histologic subtypes. To mitigate this challenge, our investigation delves into the potential utility of 68Ga-fibroblast activation protein inhibitor (FAPI) PET/CT as an alternative for staging ILC, aiming to address a significant research gap using a more expansive patient cohort than the smaller samples commonly found in the existing literature. Methods: In this retrospective analysis, women diagnosed with primary ILC of the breast underwent both 18F-FDG PET/CT and 68Ga-FAPI PET/CT. Both modalities were compared across all lesion locations with the used reference standard. The interval between scans was 1 wk, without any intervening treatments. Lesions were categorized visually, and tracer activity was analyzed using SUVmax, tumor-to-background uptake ratio, and uptake ratios. Both modalities were compared across various parameters, and statistical analysis was performed using SPSS 22.0. A P value of less than 0.05 was chosen to determine statistical significance. Results: The study included 23 female ILC patients (mean age, 51 y) with hormone-positive, human epidermal growth factor receptor type 2-negative tumors. Most (65%) had the luminal A subtype. 68Ga-FAPI PET/CT outperformed 18F-FDG PET/CT, with higher tumoral activity and tumor-to-background uptake ratios (P < 0.001). Primary tumors showed significantly increased uptake with 68Ga-FAPI PET/CT (P < 0.001), detecting additional foci, including multicentric cancer. Axillary lymph node metastases were more frequent and had higher uptake values with 68Ga-FAPI PET/CT (P = 0.012). Moreover, 68Ga-FAPI PET/CT identified more lesions, including bone and liver metastases. Pathologic features did not significantly correlate with imaging modalities, but a positive correlation was observed between peritumoral lymphocyte ratio and 68Ga-FAPI PET/CT-to-18F-FDG PET/CT uptake ratios (P = 0.026). Conclusion: This study underscores 68Ga-FAPI PET/CT's superiority over 18F-FDG PET/CT for ILC. 68Ga-FAPI PET/CT excels in detecting primary breast masses, axillary lymph nodes, and distant metastases; can complement 18F-FDG PET/CT in ILC; and holds potential as an alternative imaging method in future studies.

A comparative study between the diagnostic performance of contrast-enhanced digital mammography and dynamic contrast-enhanced MRI in invasive lobular carcinoma of the breast

BackgroundInvasive lobular carcinoma is the second most prevalent histological subtype of breast cancer after invasive duct carcinoma, with a reported increased incidence in the last two decades. It often presents with challenging imaging characteristics that lower the sensitivity of mammography in their detection and delineation of their extent. Moreover, an increased risk of having synchronous lesions in the same or opposite breast was reported in cases with invasive lobular carcinoma. This obviates the need for other imaging modalities, specifically contrast-enhanced imaging modalities, to improve early detection as well as allow precise determination of the extent of the disease. Our aim in this study was to compare the diagnostic performance of contrast-enhanced digital mammography and dynamic contrast-enhanced magnetic resonance imaging (MRI) in the local staging of invasive lobular carcinoma regarding the size, extensions, multiplicity and bilaterality.MethodsThis study included 46 female patients with pathologically proven invasive lobular carcinoma. They underwent full-field digital mammography with a complementary ultrasound examination, contrast-enhanced digital mammography, and dynamic contrast-enhanced MRI. The findings encountered by the three imaging modalities were evaluated independently, and the results were compared with final histopathology.ResultsIn the current study, dynamic contrast-enhanced MRI study was the most sensitive modality in the detection of the index lesion, synchronous ipsilateral and contralateral lesions and achieved a sensitivity of 100% in each analysis as compared to contrast-enhanced digital mammography, which achieved a sensitivity of 97.8%, 85.7% and 80%, respectively. Regarding the assessment of the lesion extent to the surroundings, there was a tendency to overestimation by MRI examination.ConclusionsAlthough dynamic contrast-enhanced MRI is the most sensitive imaging modality for detecting the index lesion, multiplicity and bilaterality, contrast-enhanced digital mammography achieved comparable overall accuracy. Regarding the locoregional staging of invasive lobular carcinoma, there was a tendency for relative overestimation by MRI examination.

The Role of Magnetic Resonance Imaging in the Preoperative Staging and Treatment of Invasive Lobular Carcinoma

IntroductionInvasive lobular carcinoma (ILC) is known for its diffuse growth pattern and its associated challenges in diagnosing. Magnetic resonance imaging (MRI) is the most accurate imaging modality and might aid in improving preoperative staging compared to full field digital mammography (FFDM) and ultrasound (US), however current literature is inconsistent. The aim of this paper is to evaluate the accuracy of MRI staging compared to FFDM/US and pathology results. MethodsIn this single-centre retrospective study, all patients diagnosed with ILC between 2014 and 2019 who underwent preoperative MRI were included. Specific parameters studied were: (1) the need for second-look targeted biopsies, (2) detection of new tumors (ie, contralateral or multifocal), (3) changes in cTNM-classification, and (4) impact on final treatment plan. Bland-Altman plots were used to compare the tumor sizes measured on MRI and FFDM/US with actual pathological tumor sizes. ResultsNinety-nine patients were included. After performing preoperative MRI, 9 (9.1%) multifocal tumors were diagnosed after additional biopsies. Contralateral tumors were detected twice (2.0%) and cN classification was upgraded in 7 cases (7.1%). Surgical treatment or neoadjuvant treatment plans were changed in 16 patients (16.1%). Compared to histopathological results, FFDM/US underestimated tumor size with a mean of 0.4 cm (Limit of agreement (LoA): -2.8 cm to 2.0 cm) whereas MRI overestimated tumor size with a mean of 0.6 cm (LoA: -1.9 cm to 3.0 cm). ConclusionsIn our study, mean differences in tumor size measurements using FFDM/US and MRI were comparable, with similar random errors. MRI correctly diagnosed multifocal and contralateral tumors more often and provided a better cN staging.

The role of contrast-enhanced mammography in the preoperative evaluation of invasive lobular carcinoma of the breast

To assess the performance of contrast-enhanced mammography (CEM) in the preoperative staging of invasive lobular carcinoma (ILC) of the breast. The present study was a multicentre, multivendor, multinational retrospective study of women with a histological diagnosis of ILC who had undergone CEM from December 2013 to December 2021. Index lesion size and multifocality were recorded for two-dimensional (2D) mammography, CEM, and when available magnetic resonance imaging (MRI). Comparison with histological data was undertaken for women treated by primary surgical excision. Pearson correlation coefficients and Bland-Altman's analysis of agreement were used to assess differences with a significance level of 0.05. One hundred and fifteen ILC lesions were included, 46 (40%) presented symptomatically and 69 were screening detected. CEM demonstrated superior sensitivity when compared to standard mammography. The correlation between the histological size measured on the surgical excision specimen size was greater than with standard mammography (r=0.626 and 0.295 respectively, p=0.001), with 19% of lobular carcinomas not visible without a contrast agent. The sensitivity of CEM for multifocal disease was greater than standard mammography (70% and 20% respectively, p<0.0001). CEM overestimated tumour size by an average of 1.5 times, with the size difference increasing for larger tumour. When MRI was performed (n=22), tumour size was also overestimated by an average of 1.3 times. The degree of size overestimation was similar for both techniques, with the tumour size on CEM being on average 0.5 cm larger than MRI. CEM is a useful tool for the local staging of lobular carcinomas and could be an alternative to breast MRI.

Magnetic resonance imaging in the preoperative evaluation of invasive lobular carcinoma; a useful investigation?

Aim: Quantify the added benefit of preoperative magnetic resonance imaging (MRI) staging of invasive lobular carcinoma of the breast (ILC) in addition to conventional imaging with ultrasound and mammography (US/MMG). Methods: Retrospective study ILC patients at our center to investigate effect of MRI in detecting additional disease, as well as effects on subsequent surgical management, reoperation rates and disease recurrence. Results: Preoperative MRI detected additional disease in 30.4% and altered surgical management in 39.1%. MRI better predicted histopathological lesion size but did not significantly alter reoperation or recurrence rates. Conclusion: MRI is superior in ILC detection and upstages a significant proportion of patients when used in preoperative staging. The impact on surgical outcomes warrants further investigation in larger prospective studies.

The performance of contrast-enhanced mammography and breast MRI in local preoperative staging of invasive lobular breast cancer

PurposeBreast MRI is considered the best modality for preoperative staging of invasive lobular carcinoma (ILC). However, contrast-enhanced mammography (CEM) shows comparable diagnostic performance to MRI, but evidence of CEM’s accuracy in women diagnosed with ILC is scant. We aimed to retrospectively evaluate CEM and MRI accuracy in preoperative staging of ILC. MethodsILC cases diagnosed between 2013 and 2021 were collected. For both modalities, tumour diameter was extracted from the reports. Bland-Altman plots were used to assess discrepancies between size measurements according to imaging and histopathological findings. CEM and MRI’s ability to detect multifocal/contralateral cancer was expressed as sensitivity, specificity, and diagnostic odds ratios (DORs). Pairwise comparison of women undergoing both CEM and MRI was not performed. Results305 ILC-cases fulfilled preset inclusion criteria. Mean age was 63.7 years. Preoperative staging was performed using MRI or CEM in 266 (87.2%) and 77 (25.2%) cases, respectively. MRI and CEM overestimated tumour size by 1.5 and 2.1 mm, respectively. Sensitivity to detect multifocal disease was higher for MRI than for CEM (86% versus 78%), but specificity was lower for MRI (79% versus 92%). For detection of contralateral breast cancer, sensitivity for MRI was 96% versus 88% for CEM, and specificity was 92% and 99%, respectively. For both indications, DOR was higher for CEM, but differences were non-significant (p = 0.56 and p = 0.78). ConclusionCEM and MRI overestimate ILC size with comparable systematic and random errors. MRI’s higher sensitivity for detection of multifocal/contralateral cancers is accompanied by lower specificity, but discriminative ability for both modalities was non-significant.

Analysis of stage and clinical pathological characteristics of invasive lobular carcinoma with the 8th AJCC Cancer Staging system

Objective To study the meaning of breast cancer staging system by AJCC eighth edition to invasive lobular carcinoma and analysis the clinical pathological characteristics. Methods According to the eighth edition of the AJCC staging to evaluate the TNM stage and prognosis evaluation of invasive lobular carcinoma cancer patient in Peking University Shenzhen Hospital from 2011 to 2016, and compared with others in clinical pathological data. Results There were 21 cases of invasive lobular carcinoma, accounting for 2.7% of all invasive breast cancer. We found that invasive lobular carcinoma shows no significant difference (P>0.05) in ages, menstrual status, molecular features and anatomic staging and prognosis staging with others; histological grade were significantly different (P< 0.05). There were significant differences in the prognosis and staging of invasive lobular carcinoma. Conclusions Eighth AJCC staging system provides a new reference for the clinical treatment of breast cancer, should be evaluated with anatomic stage. Histological grade is relatively good in invasive lobular carcinoma and the prognosis is good, needs more research to the individualized treatment of invasive lobular carcinoma. Impact of molecular and histological subtype of breast cancer on 18FDG-PET/CT imaging: Knowledge gained from recent studies

Over the past few years, several studies have focused attention on the impact of breast cancer (BC) histological subtype or BC phenotype, as defined by hormone receptors (HR) and HER2 status, on the results of FDG-PET/CT at staging, or during neoadjuvant chemotherapy (NAC). At staging, sclerotic bone metastases from invasive lobular carcinoma (ILC) demonstrated low or no FDG uptake in comparison to metastases from invasive ductal carcinoma (IDC). The CT component of PET/CT imaging should be carefully analyzed in the staging of ILC. In patients with triple negative or HER2-positive tumors, the proportion of extraskeletal metastases is high; this must be taken into account in the diagnostic strategy. Staging based on PET/CT findings offers higher prognostic stratification value than that defined by conventional imaging workup. The yield and prognostic information are high in patients with clinical stage IIB or higher. Moreover, the intensity of tumor FDG uptake at baseline may have prognostic value for recurrence, with stronger evidence in HR-positive/HER2-negative phenotype. In the assessment of tumor response to NAC, the metabolic response, generally based on the change in SUVmax (ΔSUVmax), depends on the BC subtypes. In triple negative BC, a good metabolic response early during NAC has been shown to be predictive of pCR, and the predictive value was reinforced by combining ΔSUVmax and EGFR status. In 171 patients, no correlation was found between some recently developed PET-derived parameters, i.e. tumor heterogeneity or textural analysis, and BC subtypes. Whether these parameters offer any advantage compared to SUV measurements remains to be demonstrated.

Lobular breast cancer series: imaging.

The limitations of mammography in the detection and evaluation of invasive lobular carcinoma (ILC) have long been recognized, presenting real clinical challenges in treatment planning for these tumors. However, advances in mammography, ultrasound, and magnetic resonance imaging present opportunities to improve the diagnosis and preoperative assessment of ILC. The evidence supporting the performance of each imaging modality will be reviewed, specifically as it relates to the pathology of ILC and its subtypes. Further, we will discuss emerging technologies that may be employed to enhance the detection rate and ultimately result in more effective screening and staging of ILC.

Effectiveness of Breast MRI and (18)F-FDG PET/CT for the Preoperative Staging of Invasive Lobular Carcinoma versus Ductal Carcinoma.

PurposeWe evaluated the utility of magnetic resonance imaging (MRI) and 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) for the preoperative staging of invasive lobular carcinoma (ILC) of the breast and compared the results with those of invasive ductal carcinoma (IDC).MethodsThe study included pathologically proven 32 ILCs and 73 IDCs. We compared clinical and histopathological characteristics and the diagnostic performances of MRI and 18F-FDG PET/CT for the primary mass, additional ipsilateral and/or contralateral lesion(s), and axillary lymph node metastasis between the ILC and IDC groups.ResultsPrimary ILCs were greater in size, but demonstrated lower maximum standardized uptake values than IDCs. All primary masses were detected on MRI. The detection rate for ILCs (75.0%) was lower than that for IDCs (83.6%) on 18F-FDG PET/CT, but the difference was not significant. For additional ipsilateral lesion(s), the sensitivities and specificities of MRI were 87.5% and 58.3% for ILC and 100.0% and 66.7% for IDC, respectively; whereas the sensitivities and specificities of 18F-FDG PET/CT were 0% and 91.7% for ILC and 37.5% and 94.7% for IDC, respectively. The sensitivity of 18F-FDG PET/CT for ipsilateral lesion(s) was significantly lower in the ILC group than the IDC group. The sensitivity for ipsilateral lesion(s) was significantly higher with MRI; however, specificity was higher with 18F-FDG PET/CT in both tumor groups. There was no significant difference in the diagnostic performance for additional contralateral lesion(s) or axillary lymph node metastasis on MRI or 18F-FDG PET/CT for ILC versus IDC.ConclusionThe MRI and 18F-FDG PET/CT detection rates for the primary cancer do not differ between the ILC and IDC groups. Although 18F-FDG PET/CT demonstrates lower sensitivity for primary and additional ipsilateral lesions, it shows higher specificity for additional ipsilateral lesions, and could play a complementary role in the staging of ILC as well as IDC.

Early diagnosis of invasive lobular breast carcinoma by physical examination and mammography is often difficult. It is thus often diagnosed only at an advanced tumor stage. Sonography of the breast can increase the sensitivity for diagnosis of invasive lobular breast cancer, but this modality has its limitations even when performed by experienced investigators. The combination of mammography and sonography also has limitations, particularly with regard to the staging of the invasive lobular cancer, as tumor dimensions are often underestimated and multifocality or multicentricity often not recognized. At present, MR mammography is the most sensitive modality for the diagnosis and staging of invasive lobular breast cancer. It has therefore been recommended for local staging of invasive lobular carcinoma in the updated stage 3 guidelines for the early detection of breast cancer in Germany 2008.

Utility of ultrasound and fine-needle aspiration biopsy of the axilla in the assessment of invasive lobular carcinoma of the breast

Background The unique growth pattern of invasive lobular carcinoma (ILC) poses a challenge for preoperative assessment of disease extent within the breast. Whether it similarly limits lymph node staging by ultrasound (US) and fine-needle aspiration (FNA) biopsy was the subject of the current study. Methods A total of 217 patients with ILC who underwent axillary US were reviewed. FNA biopsy was performed when US findings were suspicious or indeterminate. Findings were compared to literature reports of US in invasive ductal carcinoma (IDC) patients. Results Axillary US was negative in 137 patients (63%) and suspicious or indeterminate in 80 patients (37%). FNA biopsy was positive in 62% (47/76 patients). Preoperative US and FNA biopsy identified 43 of 111 (39%) node-positive patients. Sensitivity of US with FNA biopsy correlated with primary tumor and nodal metastasis size. Similar results were seen in IDC populations. Conclusion US with FNA biopsy appears to be similarly useful in axillary staging of ILC and IDC patients.

Increased incidence of lobular breast cancer in women treated with hormone replacement therapy: implications for diagnosis, surgical and medical treatment

A growing body of evidence support the association between the use of hormone replacement therapy (HRT) and a higher risk of both invasive lobular carcinoma (ILC) and invasive ductal-lobular mixed carcinoma (IDLC). Overall biological and clinical features of ILC entail a more cautious diagnostic and therapeutic approach as compared with invasive ductal carcinoma (IDC). ILCs are more frequently multifocal, multicentric and/or bilateral. Mammography and ultrasound show, therefore, significant limitations, while the higher sensitivity of magnetic resonance imaging in the detection of multifocal and/or multicentric lesions seems to improve the accuracy of preoperative staging of ILCs. Early diagnosis is even more challenging because the difficult in the localization and the sparse cellularity of lobular tumours may determine a false negative core biopsy. ILC is characterized by low proliferative activity, C-ErbB-2 negativity, bcl-2 positivity, p53 and VEGF negativity, oestrogen and progesterone positive receptors, low grade and low likelihood of lymphatic-vascular invasion. However, this more favourable biological behaviour does not reflect into a better disease-free and overall survival as compared with IDC. Since lobular histology is associated with a higher risk of positive margins, mastectomy is often preferred to breast conservative surgery. Moreover, only few patients with ILC achieve a pathologic response to preoperative chemotherapy and, therefore, in most patients mastectomy can be regarded as the safer surgical treatment. The preoperative staging and the follow-up of patients with ILC are also complicated by the particular metastatic pattern of such histotype. In fact, metastases are more frequently distributed to the gastrointestinal tract, peritoneum/retroperitoneum and gynaecological organs than in IDC.

Breast MRI in invasive lobular carcinoma: diagnosis and staging

Purpose: To assess the use of breast MRI for the diagnosis and staging of invasive lobular carcinoma and its impact on surgical management.

Mammographic detection and staging of invasive lobular carcinoma

The aim of the study was to evaluate mammography in detecting and staging of invasive lobular carcinoma (ILC) in order to assess the performance and impact of observer variability. Forty-two cases of ILC were retrospectively evaluated twice by two breast radiologists. Mammographic performance as well as intra- and interobserver variations was evaluated. Thirty-five percent to 37% of the cases were understaged. The largest differences between radiologists were found in the breast imaging reporting and data system (BIRADS) classification and staging performance. These results can have serious influence on patient management.

Purpose To assess the use of breast MRI for the diagnosis and staging of invasive lobular carcinoma and its impact on surgical management. Materials and methods Retrospective study of 35 invasive lobular carcinoma, histologically diagnosed in 34 patients who underwent clinical exam, mammography, ultrasonography and magnetic resonance imaging. Results Enhancement at MRI was seen for all 35 cancers. It was focal for 24 patients, regional for10 and diffuse for 1. The kinetic of the enhancement was characteristic of malignancy for 33 patients. For 11 patients the MRI staging was positive, finding 8 news cancers. For 3 patients MRI lead to biopsy of benign lesions but improved the surgical management in the 8 cases of new cancers: wider excision for 3 cases, planned breast conservation converted to mastectomy in 3 cases and excision of contralateral lesion in 2 cases. Conclusion Breast MRI is useful in diagnosis, staging and surgical management of invasive lobular breast carcinoma.

Lobular carcinoma in situ and invasive lobular cancer of the breast

The incidence of lobular carcinoma in situ and invasive lobular carcinoma of the breast is increasing. Recent data suggest that lobular carcinoma in situ is an indolent precursor for breast cancer, rather than a pure risk factor. This could imply free surgical margins become important. The risk of contralateral carcinoma and of multifocality of invasive lobular carcinoma is higher than for invasive ductal carcinoma. Therefore, the need for mastectomy, or even for preventative contralateral mastectomy is questioned. Conventional mammography or ultrasonography cannot always give useful preoperative information about the extent of lobular cancers. The value of dynamic contrast-enhanced magnetic resonance imaging needs to be established for these patients. The risk of invasive carcinoma after lobular carcinoma in situ is increased. Invasive carcinoma is usually located at the index point of lobular carcinoma in situ and is of lobular histology. Dynamic contrast-enhanced magnetic resonance imaging can be useful in the detection and preoperative staging of invasive lobular carcinoma. The risk of local recurrence is high in patients with invasive lobular carcinoma. Mastectomy and breast reconstruction could be an option in selected patients. The response to preoperative chemotherapy is worse for invasive lobular carcinoma compared with invasive ductal carcinoma, with a greater need for rescue mastectomy. Lobular carcinoma in situ and invasive lobular carcinoma are different entities from ductal carcinoma in situ and invasive lobular carcinoma. Their biological profile should be studied further in order to make the fine tuning of treatment possible.
